PART II

Select one of the following four case scenarios. Identify the prompt in the subject line of your post, for example, Case Scenario 1 – Kiran

Complete all Part II elements of the discussion:

  • Each prompt has additional key questions to address in the body of your response to ensure an understanding of learning objectives.
  • Use at least one scholarly source other than your textbook to connect your response to national guidelines and evidence-based research in support of your ideas.
  • In your peer replies, please reply to at least two peers who chose a different case scenario – other than the one you selected. For example, if you selected Case Scenario 1, then reply to one peer who selected Case Scenario 2 and another peer who selected Case Scenario 3 or 4.
  • Be prepared to answer additional questions relating to a secondary diagnosis or consideration posed by faculty.

Case Scenario 1: Kiran is a 17-year-old adolescent female who recently immigrated to the United States with her 1-month-old daughter and 18-month-old son. Her parents are struggling to find work, and Kiran has come to the clinic to have her children checked for well-baby exams and immunizations. Her mother accompanies her to the clinic but does not speak English. Kiran has a heavy accent, but you can understand almost everything she says. Given all the family stressors, she asks that you check her children to ensure they are reaching developmental milestones.

  • What education and anticipatory guidance should be provided to Kiran about developmental milestones related to her 1-month-old daughter and 18-month-old son?
  • What key aspects of the family health assessment should the APRN focus on first?
  • What are a few key questions that would demonstrate an APRN’s understanding of cultural humility and competence?

Case Scenario 2: Allen is a 14-year-old male who has come into the clinic for a complete physical exam. Two years ago, he was placed in foster care, but he has recently moved into his paternal grandfather’s home. His grandfather is with him today. During the visit, you will perform an adolescent health history focusing on his current development and anticipatory guidance needed.

  • Allen is in what phase of adolescence?
  • Summarize the physical, cognitive, and social/emotional challenges and anticipatory guidance to prepare Allen and his grandfather for this phase of adolescence.
  • What strategies should the APRN utilize to develop trust and establish confidentiality during Allen’s history and physical exam?

Case Scenario 3: Mrs. Obi comes to the clinic today with her 2 year old son Max. She is concerned because although Max says a few recognizable sounds such as “ba” for bottle he mostly gestures and points to what he wants when he tries to communicate his needs. In addition, she is requesting information about starting toilet training.

  • What type of screening tool(s) should the APRN use to assess Max’s language and social delay and why?
  • What potential risk factors (e.g., prenatal, pregnancy, neonatal) could place Max at risk for language/social delay?
  • What type of anticipatory guidance should the APRN provide to support Max’s communication and language development?
  • How should toilet training readiness be assessed?
  • What anticipatory guidance should be provided regarding toilet training?

Case Scenario 4: Mr. Zee presents to the clinic with his son Eric for a 14-month well-child visit. He expresses concern that Eric is not walking yet. The APRN reviews Eric’s past medical and birth history in the electronic medical record. Max was born at 34.5 weeks’ gestation via emergency C-section and was discharged 2 weeks later.

  • What fine motor and gross motor skills should the APRN expect a 15-month-old toddler to perform? Should Eric be able to perform those skills as well? Why or why not?
  • What type of screening test(s) should the APRN utilize to assess Eric’s current motor skills?
  • What type of anticipatory guidance and support should the APRN provide to monitor and encourage Eric’s continued physical development?
